Changing the production function of diagnostic tests
Last week while reading Science I noticed a short and crucial article. Up to now CRISPR technology was focused on gene editing, now we can say that its usefulness is widening into diagnostics. It may change completely molecular diagnostics of "infectious diseases through detection of Zika virus (ZIKV), Dengue virus (DENV), and human papillomavirus (HPV) in human samples, and noninfectious diseases, such as detection of gene mutations in circulating cell-free DNA from lung cancer patients." The production founction of lab testing would change completely.
Several articles explain details about it. The fight for patents is going to start again on CRISPR diagnostics. And this are unfortunately bad news.
Anyway, Science article reminds us:
These emerging diagnostic tools will by necessity be compared to standard diagnostics to ensure sensitivity and specificity and will need to be field-tested to guarantee performance in patient care settings, as environmental conditions and end-user application might affect performance. Proven assays, if affordable, promise to improve care in resource-limited settings where undifferentiated febrile illness is the norm and where gaps or delays in diagnosis, targeted care, and infection control contribute to infectious disease mortality and spread.More details in The Verge.

Product Hopping in the Drug Industry — Lessons from Albuterol
Aquesta és una història que lliga el clima, la tecnologia i la big pharma. Va de l'albuterol, l'inhalador per MPOC i asma. La indústria farmacèutica va aprofitar la prohibició dels clorofluorocarbons en aquell moment que afectaven la capa d'ozó, i va capgirar el que era la patent sobre una molècula, amb patent sobre un procés. I va canviar-ho per inhaladors que contenien hidrofluoroalcà. El resultat és que fins el 2020 no hi va haver genèric. La patent del ventolin havia caducat el 1989, i la del proventil el 1997. El gràfic, referit a les vendes als USA ho diu tot.
Sorprèn que les lleis de patents acabin produint aquests disbarats i ho fan a la llum de tots plegats. Ens ho hauríem de fer mirar seriosament. Els autors ho diuen:
We worry that without patent and regulatory reform, this pattern is likely to be repeated. The history of albuterol over the past 40 years offers a cautionary tale for regulators and policymakers seeking to ensure access to prescription drugs while still meeting other goals such as environmental protection.

Operationalising Value Based Pricing of Medicines: A Taxonomy of Approaches
Observo un nerviosisme farmacèutic intens. Farmàcies que diuen que no els surten els números, farmacèutiques que diuen que no podran investigar. No discutiré en detall cap dels arguments. Només hi ha una qüestió fonamental: fer els deures en cada moment, ho he dit en altres ocasions. Els farmacèutics han tingut ocasió de plantejar reformes i han preferit l'statu quo. Algunes farmacèutiques s'han oblidat intencionadament que amb els medicaments genèrics l'argument de la incapacitat de recerca s'ha esvaït fa dies, i si no perquè hi ha patents?. Alhora miro la darrera dècada d'innovació farmacèutica al NRD de juliol i no sé veure res d'origen proper en molècules first in class. I si algú vol saber el motiu perquè la R&D ha disminuït, només ha de llegir en LaMattina a NRD d'agost. Més clar impossible, res a veure amb els preus. Sap del que parla, va ser un insider fins que es va torçar Torcetrapib i aquell matí fatídic tot s'en va anar en orris. I si encara voleu comprovar com el líder del mercat disminuirà 2.400 milions de dolars en recerca en dos anys aneu a l'epíleg d'aquest article, res a veure amb els preus.
Els deures passen per comprendre que el mercat farmacèutic ha sofert una transformació fonamental i alhora previsible. I tant previsible com era que tothom mirava a una altra banda.
Els que si fan els deures al mes d'agost són els d'OHE que acaben de publicar un informe sobre "value-based pricing" dels medicaments. A la p.23 i 24 trobareu el resum del que està passant al món.
Mentrestant per aquí veig molta gent de vacances i al regulador potser li hauria calgut allargar-les, llegir aquest informe i comprendre millor els problemes abans d'oferir la solució, com deia diumenge.

PS. Recupero una nota escrita el 2007 sobre Torcetrapib. El 30-N (de 2006) marca un punt d'inflexió, un abans i un després en pharma quan LaMattina es va aixecar al matí i en roda de premsa va dir als inversors interessats pel futur de Pfizer: Torcetrapib: The Most Important New Development in Cardiovascular Medicine in Years. We Are We Are First- n-Class and Best-In-Class (http://media.pfizer.com/pfizer/download/investors/presentations/LaMattina_Intro_112906_part1.pdf) (com us podeu imaginar, a hores d'ara aquest link no porta enlloc, si voleu el pdf podeu demanar-me'l). Era un dijous i van faltar dos dies per reconèixer que havien mort 83 persones en l'assaig clínic (un 60% més de morts respecte atorvastatina). El president de la companyia va anunciar que Pfizer abandonava la investigació sobre Torcetrapib. La importància de la qüestió és perquè aquest medicament havia de substituir a Lipitor, el de major facturació. El que esperava ser una facturació anual de 3 miliards, es va convertir en una caiguda en borsa de 25 miliards de $. En David Cutler ho ha explicat prou a NEJM.

Les bones intencions i la dura realitat
Pel que fa a l’assistència sanitària pública, abans de finançar una nova prestació caldria preguntar-se en primer lloc si és efectiva. Si pot assolir allò que pretén i en la mesura que diferents prestacions o productes “competeixen” per satisfer una necessitat, decidir socialment la cobertura només d’aquella que mostra una efectivitat contrastada. Tot això seria relativament senzill si la informació fos acurada i fidel. La segona qüestió seria revisar-ne el cost, i comparar el cost-efectivitat de la prestació/producte en relació als rivals.
Malgrat que els criteris anteriors semblen de sentit comú, l’estratègia més exitosa del moment de la indústria farmacèutica és iniciar el registre d’un medicament per a una indicació i després estendre’l a d’altres que en alguns casos no mostren cost-efectivitat. Si un medicament ja s’ha incorporar a les prestacions públiques la dificultat de vetllar per tal que s’utilitzi en les indicacions acurades és extraòrdinària. És el cas per exemple de la notícia d’avui, quan Zeltia vol estendre la indicació de Yondelis cap a càncer d’ovari. En el cas que fos certa l’efectivitat, hauria de ser necessari avaluar-lo novament, però això ja no passa mai. Una vegada ha entrat una prestació al catàleg s’utilitza per a totes les indicacions sense contrastar-ne el seu cost-efectivitat.
Alhora hi ha una altra vessant menys explícita i que representa un avantatge competitiu deslleial respecte als innovadors convencionals. Per exemple, el cas de Yondelis s’ha registrat com medicament per a malalties rares, que afecten a menys de 5 de cada 10.000 europeus. Amb això pot tenir una exclusivitat en el mercat de 10 anys després de l’autorització de comercialització, una protecció més gran que l’habitual de les patents. Si s’estén cap a altres indicacions, aquesta exclusivitat no hauria de ser aplicable. Malauradament això tampoc és possible fer-ho complir.
Vist tot plegat, una estratègia clau per la indústria és registrar els medicaments com orfes, i després explicar que tenen indicacions per a població general. És una forma de blindar el mercat i crear monopolis antinaturals. Algú s’hauria de preguntar si això és el que volem com a ciutadans o altrament que els recursos provinents dels nostres impostos s’utilitzin acuradament i per tant assolint el màxim valor. El criteri de cost-efectivitat hauria de ser un referent per a tota indicació sigui del medicament que sigui, orfe o no.
Mentrestant, l’anunci de l’agència britànica NICE de no admetre determinats medicaments existents per a la indicació de cancer de ronyó avançat pot ser un nou exemple de controvèrsia regulatòria. Els medicaments ja són al mercat, qui vetllarà per tal que no es dispensin als 1700 britànics que tenen la malaltia?. Si els recursos són limitats, les decisions que cal prendre han de ser aplicables. Les bones intencions són insuficients davant la dura realitat del moment.

Acaba de publicar-se un informe de LSE per a la Health Foundation sobre política farmacèutica en tres volums. Es tracta d'un text convencional, introductori per qui vulgui endinsar-se en la qüestió. Per tant com a descripció serveix. Però sobretot serveix per al debat que comença ara a UK sobre com regular la indústria farmacèutica. Al darrera hi trobem un think tank, la Health Foundation.
Val la pena fer-hi una ullada. Ara bé, si considerem el que està passant al sector de veritat veurem que sobrevola una altra realitat paral·lela que ja he recollit fa uns dies en aquest blog. Estem en moments de la màxima expressió de la teràpia farmacològica com actiu financer. És a dir que hi ha un mercat d'innovació on es decompta la rendibilitat futura de la inversió, i després hi ha un mercat farmacèutic que combina amb el mecanisme de patents per tal d'aproximar-ho al pacient. I això precisament no ho recull el document, ni tampoc la visió tradicional que es redueix en aquest gràfic prou conegut:

The forthcoming systemic drug industry?
The trend is focused towards a new industry structure after the failure of the two parts model: innovative and generic. WSJ says:
A wave of mergers and acquisitions is reshaping the global pharmaceutical industry. Many drug companies are narrowing their focus, dropping out of noncore businesses and bulking up where they have the size and expertise to generate significant sales growth.
The deals would leave fewer competitors with larger revenue streams in each segment of the drug business, from prescription medicines and vaccines to drugs for livestock and pets.After the failure of the standard innovative model throught patents, the alternative is to concentrate on rare diseases, and on highly profitable market segments -low volume and high profit-. Concentration is taking place also in commoditizated markets (generics).
Such a level of market concentration should lead to competition policy concerns, since the rivalry is not at industry level, it is at therapeutic group level. Unfortunately regulators are on vacation again. Maybe one day we will complain about a systemic industry that some of its parts may collapse and creates larger risks than returns, but it will be too late.
PS .Def: Systemic risk can be defined as the likelihood and degree of negative consequences to the larger body. With respect to federal financial regulation, the systemic risk of a financial institution is the likelihood and the degree that the institution's activities will negatively affect the larger economy such that unusual and extreme federal intervention would be required to ameliorate the effects

La recerca d'un fàrmac per a combatre l'obsesitat acumula fracassos. Si el Xenical, únic medicament reconegut, pot assolir una reducció del 5% del pes, amb el Qnexa que era el més prometedor arribava al 10%!!!. Els efectes adversos han provocat que la FDA hagi rebutjat la seva comercialització. Fa quinze dies Abbot va retirar del mercat Meridia, per problemes vasculars. Sanofi va haver de retirar Acomplia i decidir al mateix temps que Pfizer, que deixava d'investigar en aquesta qüestió.
És possible afrontar el problema des d'una altra perspectiva no farmacològica? És clar que si, però abans cal canviar algunes coses.
En primer lloc convé recordar que els medicaments obtenen patents perquè es compensa l'activitat investigadora, aquella que encerta i la que fracassa. Malauradament els incentius per acumular fracassos són massa elevats a data d'avui. Podríem pensar en una alternativa, un indicador de patent ajustada per èxits del laboratori en la seva aprovació, aquells laboratoris que persisteixen en l'error tindrien una duració de patent inferior. Evidentment, estem en lo de sempre. Caldria trobar mesures per aseparar la incertesa de l'eficiència en la recerca.
Més enllà de les farmacèutiques i les seves preocupacions per trobar nous medicaments ens cal revisar novament que pot fer la regulació governamental i la societat enfront l'obesitat. I això és el que explica l'OCDE en aquest document. Mentrestant, ens podem aplicar a nosaltres mateixos la fórmula: dieta+exercici físic , i vigilar el perímetre abdominal i la cremallera.
PS. Per cert, m'he de llegir l'article d'en Kaestner al JHE que diu que una menor densitat demogràfica a les zones metropolitanes ha impulsat l'obesitat. S'ha de veure en detall.

Statistical vs. identifiable lives
The recent Ebola evacuated case exemplifies the concept created by Shelling a long time ago, the difference of how a society allocates resources according to 2 different rules:
In 1968, in a paper about valuing ways to reduce the risk of death, Thomas Schelling1 distinguished between “identified lives” and “statistical lives.” Identified lives are the miners trapped in a mine or the child with a terminal disease—specific people who need help now. Statistical lives are those people, unidentifiable before the fact and often after as well, who will be saved by a new safety regulation, public health program, or environmental standard. Schelling observed that people seem to be willing to pay more to save an identified life: “Let a six-year-old girl with brown hair need thousands of dollars for an operation that will prolong her life until Christmas, and the post office will be swamped with nickels and dimes to save her. But let it be reported that without a sales tax the hospital facilities of Massachusetts will deteriorate and cause a barely perceptible increase in preventable deaths—not many will drop a tear or reach for their checkbooks.Really such a case goes beyond Shelling insight because of uncertainty and unavailability of effective treatment. Bioethics field has argued over what they called "rule of rescue", a different perspective of the same issue. In this respect, NICE statement helps to understand both views:
When there are limited resources for healthcare, applying the ‘rule of rescue’ may mean that other people will not be able to have the care or treatment they need. NICE recognises that when it is making its decisions it should consider the needs of present and future patients of the NHS who are anonymous and who do not necessarily have people to argue their case on their behalf. NICE considers that the principles provided in this document are appropriate to resolve the tension between the needs of an individual patient and the needs of present and future users of the NHS. The Institute has not therefore adopted an additional ‘rule of rescue.The article by Louise B. Rusell reflects precisely the theoretical and practical controversy and ends with this paragraph:
Adjustments and controversies aside, the evidence provided by VSL estimates suggests that people’s willingness to pay for statistical lives may be consistent with their willingness to pay for identified lives. The apparent existence of 2 different decision rules may have been no more than an artifact of the economic method for valuing statistical lives in use at the time the distinction was proposed. Now that economists’ methods more fully reflect “the interests, preferences and attitudes to risk of those who are likely to be affected by the decisions,” their estimates of the value of a statistical life support the idea that there just may be a single rule: Identified and unidentified lives may be equally valuable. This is good news for decision makers who use cost-benefit and cost-effectiveness analysis to inform decisions.The theoretical suggestion sounds good, nowadays the political decision making reality goes in the opposite way, at least close.

La motxilla epigenètica
La nova medicina combinarà disciplines òmiques, i això és va fer palès al curs d'epigenètica al que vaig assistir. Les dues aportacions de la Maria Berdasco van ser d'elevada qualitat, la introducció i la d'epigenètica i càncer. Vaig tenir la impressió que s'ha avançat molt en recerca i hi ha molt més del que imaginem per traslladar a la pràctica ( per exemple a la p.31 trobareu quatre medicaments ja disponibles).
Avui topo amb una mostra més de que l'epigenètica és el tema del moment al The Economist d'aquesta setmana. La troballa de Rehan et al. assenyala que l'herència epigenètica es podria traspassar dues generacions. Ho fan amb ratolins. En concret si l'àvia era fumadora, els nets mostren les alteracions epigenètiques provocades per la nicotina.
Si tot plegat va per aquí, ja cal que ens ho fem mirar. El genoma és el que és i la metilació i acetilació regulen l'expressió dels gens (epigenoma). Tenint en compte que podem ser hereus dels hàbits més o menys saludables dels progenitors, i si alhora hi afegim l'status social que ens aporten -recordeu el que vaig explicar dels macacos-, tot plegat faria un motxilla epigenètica més feixuga per uns que per altres.
PS. Encara que de genoma en tenim un, i diuen que aviat el preu baixarà a 1.000 dòlars, penseu que d'epigenomes en tenim uns 450 (!!!),- un per cada tipus de cel.lula- per tant això es complica i molt.
PS. Potser aviat veurem alguns economistes fent models d'epigenetic overlapping generations....per tal de predir la despesa...
PS. Presentacions d'interès d'Oliver Rivero i Vicente Ortún al mateix congrés Llàstima que al pdf hi han posat una marca d'aigua inoportuna, podria ser menys invalidant...
PS. Per cert, sobre els recursos públics disponibles per la ciència. Observo nirvis entre alguns científics, poden ser fonamentats. Ara bé, no he vist encara cap descripció dels resultats en termes de patents i llicències i el que ha suposat de retorn per al sector públic. Ni tant sols la més mínima preocupació per la transparència. Sovint em pregunto si ens trobem davant una altra privatització dels beneficis i una socialització del cost. Per ara tinc només evidència anecdòtica.

Massa poc, massa tard
El juliol passat ens vam reunir per debatre la prestació farmacèutica en el marc de la gestió de malalties. Vaig participar-hi i trobareu materials dins l'informe que s'acaba de publicar. El meu missatge, més enllà del disease management o del case management és que necessitem integració assistencial. Això vol dir coordinació i incentius acurats entre tots els que contribueixen a la millora de la salut individual i poblacional. Vaig insistir en qüestions que he comentat repetidament en aquest blog, però en especial en les guies de pràctica clínica individualitzades. I vaig remarcar que cal un esforç imponent per dur a terme un canvi en aquest sentit. De no fer-lo podrem dir allò de massa poc, massa tard.
PS. Hui, just eixint de casa trobe una merla i el seu bec groc i les plomes negres destaquen sobre la neu que acabe de davallar. Està desconcertada, no sap cap on va. Fet excepcional que així resta registrat al blog. Me'n vaig una mica més enllà i veig els Tres Turons nevats, paisatge encisador quan el sol vol sortir però no pot.
PS. Hi ha gent que s'escandalitza dels sous dels banquers rescatats (amb raó) però no presten cap atenció a com els diners públics s'utilitzen per subvencionar empreses privades que pretenen treure profit privat amb les patents. Socialització del cost i apropiació privada dels beneficis, aquest és el mecanisme de mercat distorsionat pels governants. I enmig de la crisi i retallades, convé dir prou encara més a aquestes coses.
És una vergonya, una indecència que es dediquin 1,4 milions d'euros (p.13 i 29) a crear un sistema de classificació de pacients quan el mercat n'està farcit d'opcions i està a punt de sortir el de codi obert que va regular l'Obamacare i al que m'he referit anteriorment en aquest blog com a standard de futur. Els responsables polítics no seran processats per això, malauradament, però avui ho dic més alt que altres dies: NO AMB ELS MEUS IMPOSTOS.

Durant la pandèmia es va produir una innovació notable a la tecnologia de proves diagnòstiques. Va passar desapercebuda per alguns, però no pels que llegiu aquest blog. Es tracta d'utilitzar CRISPR que inicialment s'ha desenvolupat per a edició genètica, per a la detecció d'àcids nucleïcs en un sol pas, i per tant també de Sars-COV-2. La dificultat d'aplicació que tenia aquella prova, que va rebre el vist-i-plau de la FDA era que el procés no es podia automatitzar, i per tant calia amplificació prèvia. Ara acaba de publicar-se la prova definitiva que pot capgirar moltes coses, i que per tant pot canviar la funció de producció dels laboratoris. La troballa d'enzims termoestables ha estat la qüestió clau per a l'èxit de l'equip de Feng Zhang.
Cal dir que la companyia rival, Mammoth Biosciences, de Jennifer Doudna, també té en marxa una prova similar: DETECTR BOOST.
I ara ja ha començat el procés de patentar enzims. Tant per una empresa com per l'altre. I això esdevé inadmisible si el que es pretén patentar la natura o variacions sobre la natura. Però ningú se n'està adonant , als USA ja s'ha fet tard, però a Europa encara hi som a temps.
N'estic segur que les patents, una vegada més, limitaran l'accés a aquesta gran innovació.

- aAssay time indicates the approximate incubation time most frequently used in the referred study (different assay times can be reported, depending on the intended sensitivity and the readout).
- bPOC compatibility indicates whether the entire assay as reported—including sample preparation (that is, crude extraction) and readout—can be performed at POC or in the field with minimal equipment.
- cLimits of detection cannot always be directly compared across studies, in particular because some studies did not report how the LOD was determined, or reported the target concentration either in the transport media of the sample or in the final reaction. The LODs shown here reflect the optimal LODs reported. In general, LODs depend on the type of input material (raw or synthetic), type of readout and incubation time.
